FINANCIAL AID PROGRAM THE HOPE CHEST
The Hope Chest Program for Medically Underserved Women provides financial assistance and relevant education and support services to low-income, under- and uninsured, underserved women in Western North Carolina who are in need of support during treatment for cancer.

The Hope Chest for Women Financial Aid program was created to provide assistance to patients with female cancers: cervical, breast, uterine, vaginal, vulvar, endometrial, or ovarian and who are in significant financial need. Through this program, The Hope Chest offers financial assistance and scholarships for classes offered through Hope-A Women's Cancer Center for patients being treated for any one of these diseases.

Once the Financial Aid application is approved, you may be reimbursed for treatment-related expenses, including those for drugs, radiation therapy, blood transfusion, transportation, short-term hotel stays when you are being treated, or classes offered through Hope-A Women's Cancer Center. The information presented here outlines the expenses paid by The Hope Chest and the steps involved in applying for and receiving assistance.

Who is Eligible for Assistance?
You are eligible for assistance through the Financial Aid Program if you meet the following conditions:
• You are a woman receiving treatment for cervical, breast, uterine, vaginal, vulvar, endometrial, or ovarian cancer.
• You are receiving treatment at a treatment center, hospital, clinic, or doctor’s office in western North Carolina (residents of Buncombe, Haywood, Henderson, Madison, Cherokee, Transylvania, Yancey, Rutherford, Polk, Jackson, Swain, McDowell counties.)
• If you receive federal, state, county or local, social security, Blue Cross Blue Shield, Medicare, or Medicaid funding, or if you have private insurance, you may receive aid for treatment-related costs that are covered by the Patient Aid Program, but are not covered through your other payment sources.

To Apply for Aid:
If you wish to receive aid, you should complete the Financial Aid Application. Upon completion of the Financial Aid Application, you and your physician should each sign the form. All signatures must be original-stamps, photocopies, or initials cannot be accepted. In addition, copies of outstanding medical bills should be submitted with your application as well as statement of last month’s income--pay stubs, etc. (for both spouses if married).

You should submit your application to The Hope Chest for Women, P.O. Box 16948, Asheville, NC 28816.

If your application is approved, you may receive up to a total of $500 per year in aid. Funds may be subject to availability. Payments will most often be made a provider rather than you. (i.e. pharmacy, doctor’s office, etc.)

The Hope Chest is able to help patients with significant financial need defray treatment-related expenses, including approved medications, radiation therapy, transportation, testing not covered by insurance or other sources. The Hope Chest would like to be able to provide financial assistance to all patients, but unfortunately, funds available through the Financial Aid program are limited and subject to availability. It is important that these limited funds be available for patients who are experiencing the greatest financial need. All decisions for reimbursements to eligible patients are at the discretion of The Hope Chest board.

If you believe you are in financial need at this time, contact The Hope Chest to request a Special Assistance application. Answer all questions, then bring or mail to The Hope Chest. Call 670-8403 if you need additional information.
